A New Chapter in Advanced Therapeutic Endoscopy in Bangladesh — Anti-Reflux Mucosectomy (ARMS)

Alhamdulillah.

I am pleased to share the beginning of Anti-Reflux Mucosectomy (ARMS) in my advanced therapeutic endoscopy practice in Bangladesh.

Two patients have already undergone ARMS successfully at a private hospital.

What is ARMS?

Anti-Reflux Mucosectomy (ARMS) is a minimally invasive endoscopic treatment used in selected patients with gastroesophageal reflux disease (GERD) or chronic acid reflux.

The procedure involves controlled mucosal resection at a selected part of the gastroesophageal junction.

Subsequent healing and scar formation are intended to strengthen the anti-reflux barrier.

The Reality in Bangladesh

For proper assessment of GERD, advanced centers often use investigations such as:

• 24-hour pH-impedance monitoring
• High Resolution Manometry (HRM)

However, access to these investigations remains limited in Bangladesh.

For this reason, introducing advanced anti-reflux endoscopic therapy in our setting requires particular attention to:

• appropriate patient selection
• detailed endoscopic assessment
• clinical judgment
• patient safety

The First Two Cases

The first two patients underwent ARMS successfully at a private hospital.

For me, this is not simply about introducing another procedure.

It is also about exploring an important question:

How can Advanced Endoscopic Therapy be integrated safely and effectively within the realities of healthcare in Bangladesh?

Building a Standardized ARMS Program

The next goal is to work toward a standardized ARMS program in Bangladesh, including:

• appropriate patient selection
• standardized procedures
• complication prevention
• regular follow-up
• outcome documentation
• research and publication
• future training opportunities for physicians

Beyond Being First

Performing a new procedure for the first time is certainly meaningful, but the more important goal is to establish it as a safe, scientific, reproducible and effective treatment for appropriately selected patients.

The first two cases may represent only a small beginning.

Inshallah, this beginning can contribute to another important chapter in Advanced Therapeutic Endoscopy in Bangladesh.

Starting something new is exciting, but taking responsibility for developing it properly is even more important.
